Now Offering Remote Patient Monitoring and Telehealth Services!

Remote Patient Monitoring (RPM) includes the use of connected electronic medical monitoring equipment such as pulse oximetry, blood pressure, glucometers, temperature and weight monitoring for individuals with certain health conditions. With regular monitoring, clients can benefit from early intervention in changes in medical status by trained clinical professions. A registered nurse will conduct an initial assessment and work with you to develop a personal plan of care, outlining all the services you will be receiving. The care team is flexible and committed to meeting your needs.

In Sholom Buildings

Sholom Home Care provides client-centered care to keep you safe in your apartment-home and to assist you in reaching your highest level of function and independence. Staff is available 24-hours a day, seven days a week at our assisted living and memory care locations.

Our registered nurse will assess your health history, confer with you and your family and coordinate with your physician to develop an appropriate plan of care to meet your individualized needs.

A licensed nurse is available at minimum 5 days per week, and 24/7 in some areas! A home health aide is available 24-hours a day, seven days a week under the direction of the nurse on-call.
