Sholom’s Response to Minnesota Department of Health Visitation Guide
As Sholom’s clinical team navigates the evolving regulatory guidelines throughout this pandemic, we thank you for your continued patience and understanding as we work diligently to develop measures to ensure the safety and well-being of our residents and staff.
All visits must be scheduled in advance in order to comply with occupancy capacity limits per Minnesota Department of Health guidelines. No drop in visits will be accommodated.
Scheduled essential caregivers and compassionate care visits are allowed at all Sholom’s locations.
If you would like to become an essential caregiver for a resident at one of Sholom’s locations, please see steps to follow below:
Step 2: To designate or become an Essential Caregiver, please complete Essential Caregiver Form and email to the appropriate contact below.
