Sholom has been recognized in our community for providing the highest quality senior care and compassionate services possible. For over 100 years, we have provided programs in the Hopkins, St. Louis Park, and St. Paul communities. Our services include home care, assisted living, hospice care, memory care, long and short-term care, aquatic care, and much more. Now, we are pleased to announce telehealth services as part of our service plan, which is helping to transform healthcare for our seniors.
Sholom Home Care is happy to partner with VitalTech, a growing market leader in Virtual Care Solutions, offering some of the most advanced elder care services in the country. In addition to all of Sholom’s important services, teaming with VitalTech provides additional means of reaching seniors right in their own homes.
VitalTech is the industry’s first fully-integrated virtual care platform. They offer remote patient monitoring (RPM) and digital care solutions that are transforming virtual health. Now, it is easy to access a telehealth doctor or schedule a telemedicine visit through VitalTech.
The last few years, Sholom has ramped up its innovation efforts across the spectrum of care we provide and this is yet another way Sholom is transforming senior care. Using VitalTech’s home care system, our seniors will be able to interact virtually with care givers to help manage such things as chronic conditions and monitor biometric readings such as weight and blood pressure to ensure real time feedback, support and care. At Sholom, we are continuing the think differently and continually find new ways to provide the best in-person and virtual care for today’s seniors.

Just wanted to drop you a note, and, again thank you for everything you & your staff do for my mom. Nothing you guys do ever goes un-noticed. When a person lives to be old, like my mom, many challenges occur, almost daily, and that’s just the way things seem to go. It’s not easy on you guys, and, also on us. But, we’re grateful that God has granted her many years, and along with my family, we just try & smother her with love and attention each & everyday, just like you guys. Anyway, in a world where many times people only hear complaints, I just want to be the opposite, and tell you how grateful we are to everyone at The Sholom Home. Wishing you & your family a wonderful, safe Holiday Weekend.

Often, when we think about technology, senior citizens are left out of the conversation. It is assumed that many do not participate in an online community, nor do they want access to all that the Internet has to offer. However, we know that more and more people over the age of 65 are connected digitally to family, friends, and many other venues on a daily basis.
As seniors often require doctor visits for various medical conditions, using telemedicine is providing seniors the ability to access their doctors in ways never before possible. Rather than stay alone at home, without answers to vital health questions, now seniors can enjoy a high-quality telehealth visit from the comfort of their home in Hopkins, MN.
Telehealth is vital to everyone’s highest quality of healthcare. There are numerous advantages to telehealth including:
A virtual care platform, now offered by Sholom through VitalTech services, helps to bring high-quality healthcare to seniors through telemedicine services.
There are so many ways that elderly patients and family caregivers can use telehealth to their advantage in Hopkins. Examples include:
Chronic Disease Management: At-home caregivers can use telehealth services to report on the condition of their loved ones as a disease is progressing over time. When you book a telehealth appointment, you are able to stay on top of dietary information, and mental and physical changes, as well as changes in medications.
Palliative Care: When an elderly patient’s condition begins to deteriorate, their caregivers now have the opportunity to use a telemedicine appointment to not only report on the patient’s condition, but to receive valuable feedback and advice from healthcare professionals.
Primary Care for the Very Frail: Often, as patients age, they have difficulties moving around, including leaving their home. Now, with the help of telehealth services, they can consult about a variety of primary healthcare issues and concerns. Accident management, medications, muscle stiffness, mobility, and more are all discussed with a healthcare professional.
Healthcare for Heart Failure: An at-home caregiver can now stay on top of a loved one’s heart condition through a telemedicine visit. They will understand the best ways of dealing with physical activity, dispensing medications, diet, and managing stress levels for anyone who has experienced heart failure episodes.
Sholom Home Care has offered Medicare-Certified and private pay home health care services for over a century. Now that we have partnered with VitalTech, these services will only improve the quality of life for our seniors in Hopkins, MN. When you or a loved one partake in a telehealth visit, your doctor can ascertain important information about your state of health. Your healthcare professionals can bring in other care teams with insight into providing you with what you need to do, or not do, to begin treatment at an earlier stage for better outcomes.
Patient care comes in many forms. Where once it was doctor visits in the office only, our telemedicine doctor visits can now be achieved digitally. You book an appointment online and receive the same care you would find when you drive to a medical facility.
Senior care is changing, and it is changing in big ways. That spells good news for every senior. No longer do you have to wonder about your health. Now, with telehealth services, there are new and better ways of living a full life. Sholom offers multi-disciplinary and a holistic approach to your good health. Telehealth services help us to serve you better in Hopkins.
Sholom, in partnership with our community, supports adults in need across the continuum of care, to live life fully where all are welcome. We offer convenient Twin Cities locations.
