  Johnson Center and Hospice Care Agency  

Sholom Community Alliance’s Johnson Hospice Care Agency is now fully licensed to offer hospice services to patients throughout the Twin Cities, with specialized resources available to Jewish patients and their families.

Hospice offers terminally ill individuals an alternative to aggressive curative treatments, with care focused on the person, not the disease. Hospice empowers individuals to live with dignity and to live life to it fullest.

Hospice care is directed by the patient’s primary care physician and provided by specially trained professionals, clergy and volunteers through a unique interdisciplinary team approach with the patient and family as the center of care. Patients can choose to remain at home – or wherever appropriate cares can be provided. The Johnson Center on the Rossy and Richard Shaller Family Sholom East Campus offers private suites designed specifically for hospice patients.

The Agency’s medical director is Dr. Barry Baines and Rabbi Shalom Resnik-Bell serves as chaplain. Program volunteers are trained specifically in the Hospice Philosophy through Johnson Hospice Care Agency. To learn more about the Johnson Hospice Care Agency, call 651-328-2091 or visit our website.

Short-Term Rehab Services 

Transitional care services in a dedicated short-term unit are provided for individuals recovering from illness, surgery, or needing intensive rehabilitation.

• Sholom’s goal is to return patients to the highest level of function in the shortest time period.
• Rehabilitation services are available in a spacious therapy department with state-of-the-art modalities

• Short-term care alternative for family members/caregivers caring for an elderly loved one in their home.
• An important support service to help reduce caretaker stress, sustain family stability, and provide an additional, supervised environment for the elderly person.

Johnson Hospice Care Agency

• Designed to bring comfort and support to residents and their families when illness or disabilities no longer respond to curative treatment.
• Working with a variety of external parties – including the private physician, hospice agencies and the family – specially trained staff provide specialized medical care and pain management hand-in-hand with compassion and dignity.
  

Memory Loss/Dementia Care

• Providing a separate, supportive environment configured and staffed for the distinctive needs of adults with moderate to advanced memory loss.
• Services, amenities and programs are specially designed to support each person’s dignity and unique needs.

Home Care
With high standards for individualized quality care, Sholom Care allows seniors to continue living independently while receiving first-rate, personal services. This in-house, licensed home care agency provides everything from health care to homemaking services, including housekeeping and laundry. Additional services are also available.

Vitality & Aquatics Center
A warm water therapy pool maintained at a soothing 94 degrees and the latest in exercise equipment make the David Feinberg Vitality & Aquatics Center the perfect place for pre-op or post-op treatment or to relieve chronic pain. Sholom Community Alliance offers adult day program services at both Sholom Homes.

The Roitenberg Family Adult Day Center was one of the original day programs introduced in the Twin Cities. The Center provides a variety of health, social and other related support services during daytime hours. We offer a secure, comfortable environment supervised by a licensed nurse and professional staff. Meals-on-Wheels
This program prepares and delivers Kosher meals directly to the homes of eligible individuals.

• Sholom Community Alliance works closely with the St. Paul Jewish Community Center and St. Paul Jewish Family Service to identify people living in St. Paul and the East Metro area.
• In Minneapolis and its surrounding area, the Alliance works in conjunction with the Minneapolis Jewish Federation and the Jewish Family and Children’s Service.

• As part of our commitment to serving the needs of all our constituents, we have bilingual staff (Russian and English-speaking) that assist in communicating with tenants, residents, Adult Day Center attendees and families.
