Short-Term Rehab Services

Transitional care services in a dedicated short-term unit are provided for individuals recovering from illness, surgery, or needing intensive rehabilitation.

  • Sholom’s goal is to return patients to the highest level of function in the shortest time period.
  • Rehabilitation services are available in a spacious therapy department with state-of-the-art modalities

Long-Term Care Services

  • For individuals requiring continuing care.

Respite Care Services

  • Short-term care alternative for family members/caregivers caring for an elderly loved one in their home.
  • An important support service to help reduce caretaker stress, sustain family stability, and provide an additional, supervised environment for the elderly person.

End of Life Care

  • Designed to bring comfort and support to residents and their families when illness or disabilities no longer respond to curative treatment.
  • Working with a variety of external parties – including the private physician, hospice agencies and the family – specially trained staff provide specialized medical care and pain management hand-in-hand with compassion and dignity.

Memory Loss/Dementia Care

  • Providing a separate, supportive environment configured and staffed for the distinctive needs of adults with moderate to advanced memory loss.
  • Services, amenities and programs are specially designed to support each person’s dignity and unique needs.

    Note: A caregiver support group for spouses, adult children and other caregivers of adults with dementia meets monthly at Sholom Home West.For more information.

Home Care
With high standards for individualized quality care, Sholom Care allows seniors to continue living independently while receiving first-rate, personal services. This in-house, licensed home care agency provides everything from health care to homemaking services, including housekeeping and laundry. Additional services are also available.

Adult Day Services

Sholom Community Alliance offers adult day program services at both Sholom Homes.

Sholom Home East’s Adult Day Center was one of the original day programs introduced in the Twin Cities’ area. The Center provides a variety of health, social, and other related support services during daytime hours. We offer a secure, comfortable environment supervised by a licensed nurse and professional staff. Treat yourself or a loved one to this opportunity to spend day hours with a diverse group of friendly people in a protective setting.

Sholom Home West’s adult day program is operated in conjunction with Jewish Family and Children’s Service of Minneapolis. AlterCare offers a full range of adult day services to residents of the general community and the Ackerberg Campus.

Meals-on-Wheels
This program prepares and delivers Kosher meals directly to the homes of eligible individuals.

  • Sholom Community Alliance works closely with the St. Paul Jewish Community Center and St. Paul Jewish Family Service to identify people living in St. Paul and the East Metro area.
  • In Minneapolis and its surrounding area, the Alliance works in conjunction with the Minneapolis Jewish Federation and the Jewish Family and Children’s Service.

Activities
Sholom Community Alliance offers its tenants and residents a wide range of activities to foster the greatest level of independence possible:

Religious and Spiritual Services

  • Within our facilities we celebrate Shabbat and the Jewish holidays.
  • Within our nursing homes, we provide the services of a full-time Rabbi.
  • We are respectful and supportive of our non-Jewish tenants, residents and families, honoring their holiday celebrations and religious traditions.

Language Support for Russian Tenants and Residents

  • As part of our commitment to serving the needs of all our constituents, we have bilingual staff (Russian and English-speaking) that assist in communicating with tenants, residents, Adult Day Center attendees and families.
